Carson Valley Health is proud to provide healthcare within the framework of a Patient Centered Medical Home. The Patient Centered Medical Home, or PCMH, is a team based health care delivery model that provides comprehensive and continuous medical care to our patients with the goal of maximizing health outcomes.
The purpose of a PCMH is to provide care for the whole person through better access, a dedicated provider team and care that is coordinated across the entire healthcare system. A medical home model makes sure that patients receive care in the right place, at the right time and in a way that best meets their needs.
As your medical home, we will provide you and your family with information, education and tools to help you tackle your healthcare needs. We are concerned about your overall health, including physical, mental and behavioral health. We are committed to providing you with the best care using the latest medical advances under the guidance of your provider.
The Epic MyChart portal is the easiest, safest and most efficient way to communicate with our office. Through the MyChart App on your mobile device or the MyChart link on any computer you can message your provider team, make an appointment and review medical records like lab and imaging results or your medication list.
Appointments can be scheduled through MyChart or by calling 775-782-1610. There are a limited number of same day appointments for each provider for acute/urgent issues. If your primary provider is not available, there may be another provider in the office that can see you on a same day basis.
If there is an urgent matter that we can't tend to on the same day then we conveniently have an Urgent Care on the first floor of the same building you see your provider. The CVH Urgent Care can also be utilized for after-hours situations that require attention.
In the event you need care outside of the office hours, the following can be done:
For life threatening emergencies, call 9-1-1 or go to the nearest Emergency Room.
For urgent issues, the CVH Urgent Care has extended weekend and evening hours or you can call the office number and the answering service will have one of our on-call providers reach back out to you.
For any non-urgent items such as medication refills, please send your provider team a MyChart message or call the office during normal business hours.
We strive to provide the most up to date clinical care for you at all times. We use evidence based guidelines in optimizing the way we manage and treat chronic and acute conditions as well as ongoing preventative health care. We strive to involve you in your care. We provide self- management tools and patient education and if that's not enough you have the option of a coordinator to help navigate your healthcare needs.
